Former SportWare participants may have to access their accounts to copy and upload physicals from last season.
- ALL SWIMMERS are required to do their IQ questionnaire and Concussion Education before the first day of practice. These forms are provided on the aktivate site and should be part of the registration process. Please make sure you complete the proper information required to play sports at EHS. Should only take a parent-swimmer 5 minutes to complete.
- Physicals are required for incoming Juniors, Freshmen and new to the District swimmers. These forms can be found online and then uploaded to your Aktivate account. Former SportWare participants may have to access their accounts to copy and upload physicals from last season.

**Ways to pay for swimming**
- Drop off a check with the bookkeeper at Eagle High School made out to Eagle High School.
- Pay the bookkeeper with a credit card (new this year), there will be a 3.5% transaction fee added to the total.
- You can also pay through www.myschoolfees.com with a credit card, but there will be a 3.5% transaction fee added to the total.
